      Certificate of Re-acquisition/Retention of Philippine Citizenship
    2. One (1) pc. 2X2 picture
  2. After submission of complete documents, claim your Clearance for Confirmation;
  3. Proceed to the Confirmation Area of the Office of Admissions and Scholarships at the Enrollment Services Hub.
